What to expect before your appointment:

We’ll ask you to arrive 15 minutes early to fill out our standard New Patient intake forms and other paperwork.

We send you easy text message reminders to confirm, get directions, ask us questions or reschedule if you need.

Please be sure to bring your latest insurance cards, a photo ID, list of your current medications and form of payment for your office co-pay (as applicable).

We’ll help you enroll in this free, online portal as the easiest way to talk to your doctor, request a refill, access records and more.

What insurance do you take? (Is mine in network?)

We take Medicare, Medicaid and most commercial insurance. We are also here to support our uninsured patients.

What will my co-pay be?

If you’re not sure, please contact your insurance company directly to find out your office visit co-pay.

 

For self-pay:
$20 office fee for medical
$30 office for dental

How can I get a price estimate?

You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

What if I don’t have insurance or can’t afford care?

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.

 

The self-pay office fee is:

How can I get help getting insurance?

Whether you’re a patient or not, you can schedule a FREE appointment with our Health & Outreach Advocates to help you enroll in most types of insurance. Includes temporary Presumptive Eligibility coverage, Hoosier Healthwise, Healthy Indiana Plan or other marketplace insurance.

How can I find transportation help to get to my appointment?

Medicaid patients can arrange transport through your insurance at least
two days in advance of your appointment by calling 855-325-7586.


For Anthem HIP patients, see if options are included in your plan by calling: 844-772-6632.


For other patients, please visit your Practice Location webpage to see available public transportation options and ask us about a bus pass (if available) if you’re in need
