Our History
2009
JPCHC is Founded
Founded with support from the Metropolitan School District of Warren Township, Community Health Network, and the Community Health Network Foundation.
2011
Awarded FQHC Status
Awarded Federally Qualified Health Center (FQHC) status by the Health Resources and Service Administration (HRSA).
2017
Awarded PCMH Recognition
Awarded Patient-Centered Medical Home (PCMH) Recognition, meeting HRSA standards.

A local community approach
We’re here for our central Indiana communities. That’s why renowned, Emmy winning TV news personality, and current host of “CBS Sunday Morning,” Jane Pauley, lends us her namesake to care for her hometown. As a longtime children’s and mental health spokesperson, she continues to support our vision for personal health and happiness for all.

The mission of The Jane Pauley Community Health Center, Inc. (Corporation) is to promote a healthy community through the provision of accessible, respectful, and integrated healthcare with local partners to any and all individuals and families, regardless of insurance status.
Our Vision
The Jane Pauley Community Health Center will build on its partnerships, collaborations, and community resources to revolutionize healthcare and dramatically improve the personal health and happiness for all residents of the JPCHC neighborhoods and the overall health of the community.
