During severe weather, JPCHC locations may close to keep our patients and staff safe until the severe weather passes.

Cold winter temperatures are dangerous after just a few minutes!  Seek a warm place at these locations. 

Indiana 2-1-1 offers a list of warming shelters throughout the state.

Are We Closed During Your Scheduled Appointment?

All patients with scheduled appointments at any JPCHC location that closes during severe weather will receive a text message with information about the appointment.

 

We can often do your appointment virtually with your provider.  Or, you may prefer to reschedule to another day.

 

You will not be charged for a missed appointment if this happens.  If you cannot reply to our messages, please call us after we reopen to reschedule your appointment at (463) 209- 8503.

We take Medicare, Medicaid and most commercial insurance. We are also here to support our uninsured patients.

You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

If you’re not sure, please contact your insurance company directly to find out your office visit co-pay.

 

For self-pay:
$20 office fee for medical
$30 office for dental

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.

Whether you’re a patient or not, you can schedule a FREE appointment with our Health & Outreach Advocates to help you enroll in most types of insurance. Includes temporary Presumptive Eligibility coverage, Hoosier Healthwise, Healthy Indiana Plan or other marketplace insurance.
