JPCHC Sliding Fee Scale (SFS) for discounted care:
No one will be denied access to services due to inability to pay. At Jane Pauley Community Health Center, we have our own internal discount program to reduce the costs of our services by 25%, 75% or 100%. This is for our uninsured or underinsured patients based on your family size, and with qualifying income.
- Patients can apply for SFS before, during or within 30 days after an appointment for coverage.
- This is specifically for your JPCHC service costs. (We do not cover services/costs outside JPCHC and advise you to check with those groups on their own programs directly.)
- Self-pay patients will need to pay a $20 service fee ($30 for dental) at the time of your appointment.
- If approved for SFS, your status is effective for one year from the date of application.
- Patients must reapply every year and are also welcome to reapply if anything changes with your income to see if you qualify for a better discount.
- Typically, we’ll let you know within a few days of submitting your completed, confirmed paperwork what you qualify for.
- Patients need to provide their total household income from the last 30 days.
- If you are unemployed, we have supporting documents you can fill out.
- You may provide any of the following for you and your family:
- 2 paycheck stubs (even if pay is b-weekly)
- If self-employed, last year’s taxes
- Unemployment compensation letter
- SSI/Disability Award Letter
- Pension Award Letter
- If unemployed, copy of WorkOne Authorization
- Copy of denial letter from Medicaid/Marketplace

Help with medication costs
Talk to your Provider about medications prescribed you cannot afford. They will connect you to our Health & Outreach Advocates (HOAs) for free support to help you apply for various assistance programs available that may be able to help.
Our health center collaborates with various pharmaceutical companies, state programs and nonprofits that have drug assistance programs (PAPs) to offer free or low-cost medicines if you don’t have insurance or are underinsured and can’t afford your medicine.
Program requirements and benefits all vary. Our Advocates can help you apply for the latest benefits based on what your Provider prescribes. You must be a patient and your Provider will need to sign off on your applications.
Please note, processing time for the prescription assistance program can sometimes take up to 4 weeks (sometimes longer with medication shipping). This is up to the manufacturers and not something JPCHC can control.
Typically, if your application is approved the medication is shipped to your health center and the HOA will contact you to let you know it’s ready for pick up.
JPCHC is a covered entity under the federal 340B Drug Pricing Program. For our uninsured patients, or those without prescription insurance coverage, your Provider may be able to enroll you in the 340B program when they send in qualifying prescriptions. You’ll get a card making you eligible for select discounted 340B Drug costs across about 100 participating pharmacy partner sites, including Walgreens.
