For Traditional Medicaid Patients:
- Non-emergency transportation is available through your insurance. – To request a ride, members should first call Southeastrans at 855-325-7586 at least two business days in advance of an appointment. (More Information)

- We are located on the third floor in the Community Health Pavilion inside the door right across from the elevator.
- Parking is available in the Community Health Pavilion lot.

You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.
