Primary Care & Family Medicine Services
- Complete physical exams: Physicals for sports, Medicare, and Medicaid disability.
- Acute health problems: Cold, flu, sore throat, sinus infections, ear infections, UTIs, pink eye, vomiting, and headaches.
- Chronic health conditions: Complete physical, emotional, and behavioral evaluations.
- Immunizations: Flu and pneumonia shots; hepatitis A and B series; and vaccinations for tetanus, diphtheria, meningitis and meningitis B, pertussis (whooping cough), measles, mumps, and rubella.
- Lab work: EKG, blood work, STD testing, and pregnancy testing.

You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

two days in advance of your appointment by calling 855-325-7586.
For Anthem HIP patients, see if options are included in your plan by calling: 844-772-6632.
For other patients, please visit your Practice Location webpage to see available public transportation options and ask us about a bus pass (if available) if you’re in need

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent pay stubs. If you aren’t working or don’t have income, simply bring your ID.

Whether you’re a patient or not, you can schedule a FREE appointment with our Health & Outreach Advocates to help you enroll in most types of insurance. Includes temporary Presumptive Eligibility coverage, Hoosier Healthwise, Healthy Indiana Plan or other marketplace insurance.
