Need help getting to your appointment?
For Traditional Medicaid Patients:
- Non-emergency transportation is available through your insurance. – To request a ride, members should first call Southeastrans at 855-325-7586 at least two business days in advance of an appointment. (More Information)

For other patients:

We take Medicare, Medicaid and most commercial insurance. We are also here to support our uninsured patients.
You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.

Whether you’re a patient or not, you can schedule a FREE appointment with our Community Resource Navigators to help you enroll in most types of insurance. Includes temporary Presumptive Eligibility coverage, Hoosier Healthwise, Healthy Indiana Plan or other marketplace insurance.
