Family Practice
Well checks and preventative services, acute issues, chronic disease management.
Pediatrics
Well-child checks, sports physicals, vaccinations, preventative and acute care. (Newborn-18)
OB/GYN
Well women’s exams, pap smears, contraception (birth control, including OCPs, IUDs, Nexplanon, Depo-Provera), STD checks, menopause and issue management, prenatal and pregnancy care, including ultrasound. (NOTE: No commercial insurance patients.)

Individual and group counseling, consultations and assessments, Registered Play Therapist™ for children’s counseling

- We are located on the third floor in the Community Health Pavilion near the corner of 21st and Shadeland.

You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.
