You have the right to a Good Faith Estimate for non-emergency services scheduled. Be sure to request at least three business days before your appointment.

For self-pay:
$20 office fee for medical
$30 office for dental
No problem. We provide services on a sliding fee scale based on your income if you can bring two recent paystubs. If you aren’t working or don’t have income, simply bring your ID.

Whether you’re a patient or not, you can schedule a FREE appointment with our Health & Outreach Advocates to help you enroll in most types of insurance. Includes temporary Presumptive Eligibility coverage, Hoosier Healthwise, Healthy Indiana Plan or other marketplace insurance.
